Returning to Exercise After Liposuction

Exercise is essential to healing and long-term results — but the wrong exercise at the wrong time undoes weeks of contour work. Follow this week-by-week protocol, and adjust to your surgeon's clearances.

Day 0–3

  • 5–10 minute walks every 2–3 hours around the house
  • Ankle pumps in bed to reduce DVT risk
  • No stair climbing beyond necessity
  • Heart rate at rest — no exertion

Week 1

  • Continue frequent short walks
  • Increase to 15–20 minutes twice daily by end of week
  • No lifting > 10 lb (a gallon of milk)

Week 2

  • Return to desk work
  • 30-minute walks at comfortable pace
  • Light stationary bike, no resistance
  • Still no gym, no lifting > 15 lb

Weeks 3–4

  • Elliptical or incline walking at moderate effort
  • Upper-body machine resistance (light, no breath-holding)
  • Continue compression during workouts
  • No abs, no burpees, no jumping

Weeks 4–6

  • Return to weight training with reduced load (60–70% of pre-op)
  • Machine-based leg work OK; free-weight squats/deadlifts at 6 weeks
  • Yoga (avoid inversions and deep twists)
  • Swimming after incisions are sealed

Week 6+

  • Full resistance training with normal load progression
  • Running, HIIT, plyometrics
  • Core work (planks, hollow holds)
  • Reformer Pilates

Warning signs to stop

Sudden asymmetric swelling, pain that is worse the day after (not just muscle soreness), a fluid pocket you can feel, or bruising that recurs. Stop training, return to compression, and contact your surgeon.

Frequently asked questions

When can I walk after liposuction?
The same day. Short, gentle walks (5–10 minutes every 2–3 hours) reduce blood clot risk and start on day 0.
When can I go back to the gym?
Light cardio at 2 weeks, machine-based resistance at 4 weeks, full free-weight and high-impact training at 6 weeks — assuming your surgeon has cleared you.
Can I do abs early?
No direct core work for 4–6 weeks. Loaded core exercise can increase swelling, delay contour, and — in combined tummy-tuck cases — stress muscle repair.
Will exercise change my results?
Yes, positively. Once cleared, strength training builds the muscle definition liposuction reveals. Weight maintenance keeps the result stable.
What about running?
Wait 6 weeks minimum. Impact before then aggravates swelling and can create seromas in the treated area.
Can I do yoga or Pilates?
Gentle yoga (no inversions or deep twists) at 3 weeks; full practice at 6 weeks. Reformer Pilates typically at 6 weeks.
What about swimming?
Only after incisions are fully sealed (usually 3 weeks) AND swelling has subsided enough to tolerate compression during the swim.
Should I wear compression during exercise?
Yes — through week 8. It supports healing tissue and reduces exercise-induced swelling.
